Request for Medical Documentation

If you need to request medical or billing records related to services from Home Care Delivered, Inc. (HCD), there are two ways to obtain the documentation. These options allow patients or authorized representatives to request records securely and in compliance with healthcare regulations.

Authorized representatives will be required to sign documentation confirming their right to act on behalf of the individual patient in accordance with HIPAA requirements.

Option 1: Contact Your Physician

Your physician may have copies of relevant medical documentation in your patient record. Contact your healthcare provider directly to request copies of the medical records they have on file.

Option 2: Request Records from Home Care Delivered

You may request medical record from Home Care Delivered by completing our Medical Records Request Form.

Steps to Request Records

1. Download the Medical Records Request Form (PDF).

2. Print the form
3. Complete the form
   Fill out all required fields to ensure your request can be processed.
4. Mail the completed form to:

Processing Time for Medical Records Requests

The time required to process requests may vary depending on the type of request, legal requirements, and contractual obligations.

Please allow up to 30 days for processing once your request has been received.

Please Note:

I understand that this Authorization will remain in effect until the earlier of (i) the term of this Authorization expires or (ii) my written notice of revocation to Home Care Delivered’s Privacy Department. The revocation will be effective immediately upon Home Care Delivered’s receipt of my written notice, except that the revocation will not have any effect on any action taken by Home Care Delivered’s in reliance on this Authorization before Home Care Delivered received my written notice of revocation.
